Parents and carers are being encouraged to start transition planning early to support successful moves for children and young people with SEND, whether they are moving to a new school or college or moving into the world of work after the summer.

Teachers and/or staff from your current school or setting and those from the new setting should talk and work together with parents and children and young people to makes these transition plans.

There are lots of positive steps you can take, like organising extra visits to the new setting, taking photos of it and looking at them regularly, using prompt cards or checklists to help children and young people remember what to do and where to go and using a ‘my new school’ booklet or transition pack.
